Important Safety Information
Are there any reasons why I should not receive any Dysport® formulation?
Do not have Dysport treatment if you: are allergic to Dysport or any of its ingredients (see the end of the Medication Guide for a list of ingredients), are allergic to cow’s milk protein, had an allergic reaction to any other botulinum toxin product, such as Myobloc®, Botox®, or Xeomin®, have a skin infection at the planned injection site, under 18 years of age, or are pregnant or breastfeeding. The dose of Dysport is not the same as the dose of any other botulinum toxin product and cannot be compared to the dose of any other product you may have used.
What precautions should I advise my injector about?
Tell your injector about all your muscle or nerve conditions, such as ALS or Lou Gehrig’s disease, myasthenia gravis, or Lambert-Eaton syndrome, as you may be at increased risk of serious side effects including difficulty swallowing and difficulty breathing from typical doses of Dysport®. Tell your injector about all your medical conditions, including: plans to have surgery; had surgery on your face; have trouble raising your eyebrows; drooping eyelids; any other abnormal facial change; are pregnant or plan to become pregnant (it is not known if Dysport® can harm your unborn baby); are breast-feeding or plan to (it is not known if Dysport® passes into breast milk). Tell your injector about all the medicines you take, including prescription and over-the-counter medicines, vitamins, and herbal supplements. Using Dysport® with certain other medicines may cause serious side effects. Do not start any new medicines until you have told your doctor that you have received Dysport® in the past. Tell your injector if you have received any other botulinum toxin product in the last 4 months; have received injections of botulinum toxin such as Myobloc®, Botox®, or Xeomin® in the past (tell your injector exactly which product you received); have recently received an antibiotic by injection; take muscle relaxants; take an allergy or cold medicine; take a sleep medicine; take aspirin-like products or blood thinners.
What are possible side effects?
Dysport® may cause loss of strength or general muscle weakness, vision problems, or dizziness within hours to weeks of taking Dysport®. If this happens, do not drive a car, operate machinery, or do other dangerous activities. Serious and/or immediate allergic reactions have been reported. They include: itching, rash, red itchy welts, wheezing, asthma symptoms, or dizziness or feeling faint. Get medical help right away if you are wheezing or have asthma symptoms, or if you become dizzy or faint. Dysport® may cause serious side effects that can be life threatening. Get medical help right away if you have any of these problems any time (hours to weeks) after injection of Dysport®: Problems swallowing, speaking, or breathing, due to weakening of associated muscles, can be severe and result in loss of life. You are at the highest risk if these problems are pre-existing before injection. Swallowing problems may last for several months. Spread of toxin effects: The effect of botulinum toxin may affect areas away from the injection site and cause serious symptoms including: loss of strength and all-over muscle weakness, double vision, blurred vision and drooping eyelids, hoarseness or change or loss of voice, trouble saying words clearly, loss of bladder control, trouble breathing, and trouble swallowing. Dysport® dosing units are not the same as, or comparable to, any other botulinum toxin product. There has not been a confirmed serious case of spread of toxin effect when Dysport® has been used at the recommended dose to treat frown lines, crow’s feet lines, and/or forehead lines. Other side effects of Dysport® include: dry mouth; discomfort or pain at the injection site; tiredness; headache; neck pain; and eye problems: double vision, blurred vision, decreased eyesight, drooping eyelids and eyebrows, swelling of your eyelids and dry eyes. A credit card is required to be placed on file to complete the booking process for your appointment. The credit card on file will not be charged to book your appointment, but will remain on file in the event of an infraction of our 24 Hour Policy listed below:
24 HOUR POLICY
River Oaks MedSpa has a 24 hour advanced notification policy which exists out of respect for our clients and staff schedules. Less than a 24 hour notice for the listed reasons below will result in a non-refundable $150 charge to the credit card you placed on file.
River Oaks MedSpa has a 15 minute Late Policy which exists out of respect for clients and staff schedules. If you arrive 15 minutes past the appointment scheduled time, you may be charged $150 to the credit card on file and may be required to reschedule to another date and time.

This may also include certain demographic information such as date of birth, age and gender.
Information related to your purchase(s) of goods and/or services through River Oaks MedSpa, such as your credit/debit card information, billing address, the goods and services purchased, and other related transaction information.
Information about your Internet connection, browser, or computer/device, IP address or device ID, and information on how you use and interact with the River Oaks MedSpa, such as pages or features you access.
SOCIAL MEDIA AND THIRD PARTY APPS:
If you link your social media profiles and accounts to River Oaks MedSpa, or otherwise interact with River Oaks MedSpa through social media networks (e.g., by clicking a Facebook “like” button), information may be made available to, and collected by, River Oaks MedSpa through those social media profiles and accounts. In addition, if you elect to connect any third-party apps (e.g., Brilliant Distinctions, Aspire) to River Oaks MedSpa in order to utilize certain features and functionalities, we may collect information about you from such Third Party Apps.
